How a NICE-Compliant Assessment Pathway Works.

The exact pathway depends on the assessment type, age, clinical presentation, available records, safeguarding considerations and whether medication or shared care may be relevant.

1 Initial Enquiry

The patient, family member or referrer submits a brief enquiry explaining the assessment need, main symptoms, age, urgency and any relevant background.

2 Suitability Review

Health Private reviews whether online assessment is suitable and whether further information, documents or referral details are needed before proceeding.

3 Background Information

Relevant documents may be requested, such as previous reports, medication history, school or occupational information, GP letters or collateral history.

4 Clinical Assessment

Dr Hashmi completes an online psychiatric assessment, exploring symptoms, history, functioning, impact, risk, co-existing conditions and diagnostic considerations.

5 Tools and Questionnaires Where Appropriate

Questionnaires, rating scales or screening tools may be used to support the clinical process, but they do not replace clinical judgement.

6 Diagnostic Formulation

The assessment findings are considered alongside clinical criteria, developmental history, functional impact, co-existing symptoms and alternative explanations.

7 Treatment and Management Plan

Recommendations may include psychoeducation, therapy, medication review, e-prescription where clinically appropriate, lifestyle strategies, reasonable adjustments or onward referral.

8 Report, Feedback or Follow-Up

Where agreed, the outcome may include a written report, feedback meeting, follow-up review, medication monitoring or shared care documentation where clinically appropriate.

ADHD Medication and Monitoring

NICE-Informed ADHD Medication Review and Stabilisation.

Where ADHD medication is clinically appropriate, Health Private may consider physical health checks, medication suitability, risks and benefits, side effects, dose response, stability and follow-up review. E-prescriptions may be arranged through an online pharmacy service where clinically appropriate.

Physical health checks: blood pressure, height, weight, blood tests or ECG may be required before medication is initiated or reviewed.

Follow-up review: medication may need monitoring to assess effect, side effects, dose suitability and stability.

Shared care: once medication is stable, a shared care request may be made to the GP, but GP acceptance is not guaranteed.

Important Medication Note

Medication is not automatic after assessment or diagnosis. Prescribing decisions are made individually by Dr Hashmi after clinical review, suitability assessment and any required physical health checks.

E-prescription may be offered where clinically appropriate.
Medication monitoring may require follow-up appointments.
Shared care is subject to GP review and acceptance.
Shared Care

Shared Care Requests Must Be Clinically Appropriate and Accepted by the GP.

In some ADHD cases, once medication is stable and clinically appropriate, Health Private may prepare documentation to support a shared care request. This may include diagnosis, medication history, reviews, monitoring information and recommendations.

Important: NHS shared care is not automatic. GP practices may accept, decline or request further information. Health Private cannot guarantee GP acceptance.
1 Clinical Stability

Medication normally needs to be stable before shared care is requested.

2 Review Evidence

The GP may expect evidence of diagnosis, specialist review, medication response and monitoring.

3 Shared Care Documentation

Health Private may provide documentation to support the shared care request where appropriate.

4 GP Decision

The GP decides whether to accept prescribing and monitoring responsibilities under shared care.
