5. Enquiries, referrals and registration

Scores and questionnaire results are intended for clinician review. Where appropriate, Health Private Ltd may choose not to show scores or interpretations automatically to the person completing the form, to avoid misunderstanding or unnecessary distress.

A clinician may consider questionnaire results alongside clinical history, presentation, risk, functioning, collateral information, records and professional judgement.

7. Appointments and assessments

Appointments and assessments may take place online, by telephone, in person, or through another agreed format.

You are responsible for:

  • Attending at the agreed time.
  • Providing accurate information.
  • Ensuring you have suitable privacy for online or telephone appointments.
  • Ensuring your internet, device, camera and microphone work for online appointments.
  • Providing requested documents in advance where applicable.
  • Informing Health Private Ltd if your circumstances change.

If an appointment cannot proceed because required information is missing, the person is unsuitable for the appointment type, there is a risk or safeguarding concern, or the clinical situation requires a different service, Health Private Ltd may rearrange, pause, decline or redirect the appointment.

8. Fees and payment

Fees may vary depending on the service requested, urgency, appointment type, document review, report complexity, professional instruction and administrative requirements.

Where a fee is fixed, it will be shown on the website or confirmed before booking. Where a fee depends on the nature of the work, Health Private Ltd may provide a quotation.

Payment may be required in advance for private appointments, online consultations, reports, forms, administrative work or other services.

For medico-legal, occupational, employer, insurer, solicitor, tribunal or court-related work, payment terms may be agreed separately.

Health Private Ltd may withhold work, reports, appointments, letters or documentation until payment has been received, unless otherwise agreed in writing.

9. Cancellation, rearrangement and non-attendance

Cancellation and rearrangement rules may vary depending on the type of service, urgency, clinician availability and whether work has already started.

Unless otherwise agreed in writing, the following general approach may apply:

  • If you cancel with sufficient notice, Health Private Ltd may rearrange or refund in line with the agreed booking terms.
  • If you cancel at short notice, an administration fee or cancellation charge may apply.
  • If you do not attend an appointment, the fee may be non-refundable.
  • If work has already started, including document review, preparation, clinical administration, professional correspondence or report drafting, Health Private Ltd may charge for work already completed.
  • For urgent appointments or reports, cancellation rights may be limited once work has started with your agreement.

Specific cancellation terms may be confirmed at the time of booking or instruction.

10. Reports and professional instructions

For medico-legal, occupational health, mental capacity, court, solicitor, employer, insurer, tribunal or other professional reports, Health Private Ltd may require:

  • A clear letter of instruction.
  • The questions to be answered.
  • Relevant records and documents.
  • Consent or authority where required.
  • Confirmation of deadlines.
  • Agreement of fees and payment terms.

Report timescales may depend on assessment arrangements, document volume, complexity, clinician availability, urgency, receipt of payment and receipt of all required information.

Health Private Ltd aims to provide clear and professional reports, but cannot guarantee that a report will support a particular legal, employment, insurance, clinical or personal outcome.

Clinical and expert opinions are provided based on the information available at the time.

11. Mental capacity assessments

Mental capacity assessments are decision-specific and time-specific. A person may have capacity for some decisions but not others.

Health Private Ltd may consider capacity-related instructions where appropriate. The assessment may depend on the specific decision, available records, clinical condition, legal context, urgency and whether the required assessment falls within the service scope.

No outcome can be guaranteed.

12. Medication reviews

Medication reviews may include discussion of current medication, side effects, treatment response, diagnosis, previous medication, physical health considerations and treatment options.

A medication review does not guarantee that medication will be started, changed, continued or prescribed.

Please do not send emergency information through email, WhatsApp or website forms.
